This Privacy Policy describes how Bay Area Professional Counseling, LLC ("we," "us" or "our"), located at 6576 Airport Blvd, Suite C200, Mobile, AL 36608, collects, uses and protects information when you visit bayareaprofessionalcounseling.com (the "Site") or contact us through it.
This policy applies to the Site only. Information about your care as a client of our practice is protected under the Health Insurance Portability and Accountability Act (HIPAA) and is governed by the separate Notice of Privacy Practices we provide at intake. Using this Site or submitting a form does not create a counselor-client relationship.
Information you give us. When you use our appointment request form, we collect the details you choose to provide: your first and last name, email address, phone number, date of birth, insurance carrier and member number, preferred therapist and any message you write. You may also contact us directly by phone or email.
Information collected automatically. Like most websites, the Site collects basic technical information such as your IP address, browser type, device type, the pages you visit and the date and time of your visit. We use Google Analytics to understand how visitors use the Site. This information does not identify you by name.
We use the information you provide to respond to your request, call you back to schedule an appointment, match you with a counselor, verify insurance benefits and communicate with you about our services. We use technical information to keep the Site secure and to improve how it works. We do not sell your personal information, and we do not use it for third-party advertising.
Our appointment request form is a convenient way to reach us, but it is not a secure clinical channel. Please limit what you share on the Site to the basics we need to call you back. We will gather any additional details, including your reasons for seeking counseling, by phone or in person.
Form submissions are stored on our website and delivered to our office by email so that we can follow up with you. Once you become a client, your clinical records are maintained separately and protected as described in our Notice of Privacy Practices.
The Site uses cookies and similar technologies for essential functions and for Google Analytics. Google Analytics places cookies to measure traffic and usage in aggregate. You can control cookies through your browser settings, and you can opt out of Google Analytics by installing the Google Analytics opt-out browser add-on. Disabling cookies may affect some features of the Site.
We share information only as needed to operate the Site and our practice: with service providers that host our website, deliver our email and provide analytics, each of which is permitted to use your information only to provide services to us; when required by law, subpoena or court order; and to protect the rights, safety or property of our clients, our staff or the public. We do not share your information with third parties for their own marketing purposes.
We keep appointment requests for as long as needed to respond to you and to maintain a record of our communications, after which they may be deleted. Analytics data is retained according to Google Analytics' standard retention settings. You may ask us to delete a form submission at any time by contacting our office.
We use reasonable administrative and technical safeguards, including an encrypted (HTTPS) connection, to protect information submitted through the Site. No method of transmission over the internet is completely secure, so we cannot guarantee absolute security. If you are in crisis or need immediate help, please do not use the Site: call or text 988, the Suicide and Crisis Lifeline, or dial 911.
The Site is not directed to children under 13, and we do not knowingly collect personal information from children through it. Appointment requests for minors should be submitted by a parent or legal guardian.
The Site links to third-party sites such as Facebook and Google Maps. Those sites have their own privacy policies, and we are not responsible for their practices.
You may contact us to review, correct or delete the personal information you submitted through the Site, or to ask questions about this policy. Requests about your clinical records are handled under our Notice of Privacy Practices.
We may update this policy from time to time. The effective date at the top of the page shows when it was last revised, and the current version will always be posted here.
